美国的牙医（Dentists）主要预防、诊断和治疗口腔和牙齿疾病，修复、矫正或拔除牙齿，制作和安装假牙，并施行口腔手术。美国的牙医绝大部分都自行开业，独立于其他医生，有完全独立的教育、分科、认证、费率、保险系统。

美国牙医的主要分科有全科牙医（General Dentist）、正齿牙医（Orthodontist）、口腔和上颌外科（Oral & Maxillofacial Surgeon）、儿科牙医（Pediatric Dentist）、牙周病牙医（Periodontist）、镶牙假牙牙医（Prosthodontist）、牙髓牙医（Endodontist）、口腔病理学医生（Oral Pathologist）、口腔和上颌放射科医生（Oral & Maxillofacial Raiologist）等。根据美国劳工部数据，2006年，美国共有161000名牙医(15)，其中全科牙医136000名，正齿牙医9200名，口腔和上颌外科医生7700名，镶牙假牙牙医1000名。牙医的平均年收入在15万～20万美元，其中的某些专科，比如正齿牙医、镶牙假牙牙医、口腔和上颌外科等平均收入在20万美元以上。牙医的收入与其他医生的收入相当，但其工作时间通常要比其他医生短，压力也没有那么大。

同其他医生一样，美国牙医必须获得所在州政府颁发的行医执照，其条件是必须从合格的牙医学院毕业，并通过资格考试。2009年，共有59所美国牙科学会（American Dental Association）认证的牙医学院，招收大学毕业生就读。如果一名大学生希望上牙医学院，那么他必须修习牙医预科课程，参加牙医学院入学考试。牙医学院的学习时间为四年，毕业之后获得口腔外科博士学位（Doctor of Dental Surgery，DDS）或口腔医学博士学位（Doctor of Dental Medicine，DMD）。获得学位和执照之后，新的牙医们就可以开业行医了。大约有11％的牙医希望继续深造，进入某个牙医专科，他们就必须申请牙医学院的研究生班，进行2～4年的专科教育，然后在所在州申请专科认证。

美国对牙医服务的保险是非常有限的。在政府提供的医疗保障计划中，成年人是不享受牙医服务的，只有21岁以下的未成年人才在医疗补助（Medicaid）计划下接受政府补助的牙医服务。有一个例外是，如果成年的非牙医治疗必须同时处理口腔方面的疾病时，政府才可以部分负责牙医服务。比如说因为口腔方面的癌症而必须拔除牙齿等情况。

雇主提供的牙医保险和其他医疗保险是分开的，雇员可以选择购买或不购买。这些牙医保险每月的保费通常比医疗保险便宜，但是自付的部分很高。雇员购买牙医保险之后，保险公司就会寄来一份费率表，标明投保人将自付的费用。同美国医学会为所有医疗服务制定代码一样，美国牙医学会制定颁布牙医操作专用代码（Current Dental Terminology），牙医的服务都有代码标示，费用也是按照代码收取，具体费率由牙医和保险公司签订的合同而定。笔者2009年初进行了臼齿根管治疗，包括牙髓清除、消毒、填充、植桩、加冠，去牙医诊所4次，一共自付费用1635美元。

由于牙医服务不属于政府福利范围，绝大多数是病人自付和雇主保险负责，因此受经济周期的影响很大。经济增长快、失业率低、劳动工资收入高时，牙医服务需求也增长；反之，经济萧条、失业率高、劳动收入低时，牙医服务的需求也减少。2008年开始的经济萧条对美国牙医行业有不利影响。
