1700637648 摘　要　血浆净化疗法通过体外循环分离、置换血浆，清除患者血浆中致病物质以达到治疗目的的一种血液净化治疗新方法。根据分离血浆的方法，分为离心法和膜式血浆分离法，膜式血浆分离法在临床上易于实施，临床应用日益广泛。血浆净化疗法临床适应证相当广泛，已应用于抗基底膜病及各种难治性肾小球肾炎、肾病综合征等肾脏疾病，此外还应用于系统性红斑狼疮、慢性及恶性类风湿关节炎等自身免疫性疾病和重症肌无力、格林—巴利综合征等免疫性神经系统疾病及重症肝炎、血液系统疾病、高脂血症的治疗等。目前新的研究和发展还在不断进行，以使这一技术更趋完善合理，今后将在临床上将发挥越来越重要的作用。

1700637650 Abstract　Plasmapheresis is a new technique to clean plasma pathogens by extracorporal plasma separation and exchange. The technique originated in the development of centrifugal devices separating plasma from cells. Subsequently, membrane filtration was developed and widely used in clinic for its facility to operate. So far, plasmapheresis was applied to treat various intractable diseases described following: kidney diseases including anti-glomerular basement membrane glomerular nephritis, intractable glomerular nephritis, nephritic syndrome et al., autoimmune diseases including systemic lupus erythematosus, chronic and malignant rheumatoid arthritis et al., neurological diseases including myasthenia gravis, acute Guillain-Barre syndrome et al., severe hepatisis as well as hyperlipidemia. Currently, plasmapheresis is a rapidly developing blood purification technique, and it would be a reasonable and perfect therapeutic way to play important role in clinical practices.

1700637654 目前，膜分离法血浆净化技术在临床上已逐步取代离心分离法，成为血浆净化治疗主要方法。膜分离法血浆净化技术主要包括单纯血浆置换（PE)、血浆成分交换（双重膜滤过法、冷却滤过法、热滤过法、肝素诱导LDL沉淀法等）和血浆吸附法（PA）三种。其基本原理是：利用一定孔径的半透膜分离血液中的有形成分与血浆，通过直接丢弃血浆（PE）或丢弃部分血浆成分（利用半透膜滤过、温度的变化、pH值的改变、特异性吸附等），以去除特定的致病大分子物质，其中大多数方法需要补充一定量的置换液（新鲜冻干血浆、白蛋白液等）。

1700637656 血浆净化疗法的临床适应证相当广泛，已应用于抗基底膜病及各种难治性或急进性肾小球肾炎、肾病综合征等肾脏疾病，系统性红斑狼疮、恶性类风湿关节炎等系统性疾病，重症肌无力、格林—巴利综合征等神经系统疾病，暴发性肝炎、手术后肝功能不全以及血液系统疾病、高脂血症的治疗等，目前也作为多脏器功能障碍的支持治疗，在感染性休克、多脏器功能衰竭以及器官移植前后的脏器支持治疗中发挥很重要的作用。目前新的研究和发展还在不断进行，以使这一技术更趋完善合理。血浆净化治疗法作为血液净化技术的手段之一，今后在临床上将发挥越来越重要的作用。
