中国医科院校毕业生取得ECFMG证书的人数和比例从1995年开始急剧下降。

在外国医学院毕业的美国公民有2064人，占总数的20.1％(19)。由于美国的医科院校入学申请竞争激烈，学费高昂，很多美国学生到外国接受医学院教育，然后再通过ECFMG认证，回到美国接受住院医生教育。这些学生大多数都在加勒比海国家接受医学院教育，这些医学院也是为了迎合这种特殊需求而开设的。

取得了ECFMG医学教育认证，国外医学院毕业生就必须与美国医科院校毕业生一起参加住院医生全国配对。2007年，在6709名非美籍外国医学院毕业生申请住院医生配对，最后有3180名获得成功，成功率为47.4％(20)。在2612名美籍外国医学院毕业生申请人中，有1347名获得成功配对，成功率为51.6％，略高于非美籍外国毕业生。同年，共有14511名美国医科院校的毕业生参加配对，其中14201名获得成功，成功率97.9％，大大高于外国医学院毕业生。这组数据显示，美国医科研究生教育机构还是偏向于接受美国医学院毕业生。

如果外国医学院毕业生不是美籍，也没有永久居留权，那么在获得住院医生配对之后，必须申请美国签证。签证有两种：J-1签证或H1-B签证。J-1签证是交流访问学者和培训签证，非移民签证的一种，到期之后必须回到所在国居住2年之后才能递交移民签证和永久居留。H1-B签证是工作签证，是移民签证的一种，在有效H1-B签证期间就可以申请永久居留。如果希望住院医师期满后在美居留行医，那么最好申请H1-B签证。

笔者所认识的国内学医再到美国做医生的朋友大都通过其他途径先到美国，有的跟随丈夫或妻子来美，有的先申请就读基础科学系的博士。在美国期间再花时间准备考试和办理各种手续。如果得到博士学位，对将来申请住院医生也是很有好处的。虽然道路比较曲折艰苦，成功者也不乏其人。

2007年，所有在美国行医的76.6万医生中，约18.4万名毕业于外国的医学院，占总数的24％(21)。在老年病专科（Geriatric Medicine）医生中占45.6％，在肾脏病（Nephrology）医生中占40％，在新生儿和围产期专科（Neonatal-Perinatal Medicine）医生中占39.9％，在内科（Internal Medicine）医生中占35.5％。但是外国医学院毕业的医生在皮肤病（Dermatology）医生中只占6.1％，在骨外科（Orthopedic Surgery）医生中只占7.2％，在急诊（Emergency Medicine）医生中只占7.7％，在眼科（Ophthalmology）医生中只占8.1％。

总体而言，美国的医生群体在种族、文化和国籍背景上是非常多元的。美国的医生在世界上以训练严、收入高而著称，因此吸引了世界各地优秀的医学人才来美国追求职业上的成功。
